If you're a new client for any service please log-in to the online portal via your portal request and fill-out all intake information and submit. If you have not received your portal invite from our office please give us a call at 509-667-0679. We ask that you arrive for your first appointment 15 minutes early and please bring your current insurance card. If you are unable to attend your appointment please call the front desk (509) 667-0679 at least 24 hours in advanced to reschedule.
If you would like us to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), please print and complete this form to authorize release of psychotherapy information:
